Dozens of vials of the Modern COVID-19 vaccine had to be discarded due to ‘human error’, Aurora Health Care officials said.
Lawyer Aurora Health officials said the incident happened over the weekend when an employee removed the vials from the Grafton Clinic refrigerator to gain access to something else and not return it.
“We have learned that about 50 scales of Moderna vaccine were accidentally removed from a pharmacy closet overnight,” a spokesman said. “While some of the vaccine was administered to team members on December 26 within the approved 12-hour window after refrigeration, most of it unfortunately had to be discarded due to the temperature storage requirements to maintain its viability.”
According to the health care system, 17,000 team members have been vaccinated in the past twelve days.
“We are clearly disappointed and regret that this has happened,” a spokesman said.
