As the deployment of the COVID vaccine continues, there are ongoing debates over whether to laminate your vaccine record or whether vaccine passports should become a requirement for access to certain sites.
In any case, your COVID vaccination card is likely to be an important document in the coming months. Travel managers said they believe people will have to show their card to fly in the future, and it is possible that some employers will also ask for proof. Some colleges already need vaccinations before returning to campus.
The Centers for Disease Control & Prevention (CDC) recommends that you take a photo of the front and back of your card, while Governor Phil Murphy advocates for the lamination of your card. future COVID booster shots.
But what happens if you lose your card?
The CDC says you should contact your vaccine administrator.
CVS, for example, said it could issue cards.
“The pharmacy where the patient received the vaccination can be issued a card again after confirming their identity with the correct identification,” said Tara Burke, spokeswoman. “The pharmacy will confirm their record on file and reissue it.”
Or you can also contact the state for a copy of your records.
“If an individual loses their COVID-19 vaccination card, they should request a copy of their vaccination record by visiting the New Jersey Immunization Information System (NJIIS),” said Dawn Thomas, spokeswoman for the Department of Health. “All doses of COVID-19 must be entered into NJIIS within 24 hours, but this can take up to 72 hours, depending on how the data is submitted in NJIIS.”
Click on the “Submit a Request” link on the top blue ribbon.
At the bottom of the page, select ‘I would like to request a copy of my vaccination record from NJIIS.’ This will open a form that you can fill out.
You only need to fill in the required fields, such as your name, address, email address and telephone number.
In the required field that asks for your “Facility / Organization Name”, you can enter “n / a”, Thomas said.
You must add a copy of your photo ID.
Acceptable forms of ID include: a state-issued photo driver’s license with address; a state-issued photo-non-manager’s identity card with address; a similar form of identification issued by this state, another state or the federal government; or a photo identity card issued by a New Jersey County Clerk, ”reads the website.
If you specifically want a copy of the lot number for your COVID-19 vaccine, add the request to the description section, the website says.
The state has not said whether some vaccination sites, such as the mega-sites, can issue maps at their physical locations.
According to the Department of Health, most requests are answered within 24 to 48 hours, and your records will be sent to you. The records could not be emailed.
