How to Apply Online for COVID-19 Vaccine Appointment in Dane County

MADISON, Wis. – Many adults turn to their children, grandchildren, and even their local librarians for help with a COVID-19 vaccination appointment. Here is a guide.

Frontline care staff, residents and staff in competent nursing and long-term care facilities, police and firefighters, correctional staff and adults aged 65 and over are all eligible to receive the COVID-19 vaccine in Wisconsin.

The Wisconsin Department of Health Services lists health care providers and pharmacies as the primary vaccines for adults 65 and older.

How to plan a vaccination appointment with a primary care physician:

Primary care physicians will be eligible for patients to plan vaccinations.

At UW Health, eligible patients will receive an invitation to schedule a vaccination appointment through MyChart (register here or sign up here). That invitation will appear on the message tab. Patients without a MyChart account will receive an invitation by phone or text message. People can also fill out a vaccine interest form and can be contacted for appointments for the same day.

At SSM Health, eligible patients will receive an invitation to schedule a vaccination appointment through MyChart (register or sign up here). Patients without a MyChart account will receive an invitation by calling Dean Medical Group, Agnesian HealthCare or Monroe Clinic. People can also fill out a vaccine interest form and will be contacted when stock is available.

According to the website, SSM Health does not plan appointments for vaccination at the first dose before 6 March.

At UnityPoint Health Meriter, eligible patients will be notified by phone, text message or email when they can schedule an appointment. According to the website, all Meriter patients aged 65 and older are offered. Meriter asks all primary care patients who have not yet been contacted to contact their primary care clinic.

At Access Community Health Centers, eligible patients would be contacted to schedule an appointment during the week of January 25th.

At Group Health Cooperative of South Central Wisconsin, eligible patients will receive an invitation to schedule a vaccination appointment through MyChart (register or sign in here) Eligible patients without a MyChart account will be emailed or emailed mail can be contacted. Patients can complete the COVID-19 vaccine interest form.

At Stoughton Health, patients who want a vaccine must complete the waiting list form. Stoughton Health will contact suitable patients to schedule appointments as doses become available.

University Health Services vaccinates employees of the University of Wisconsin-Madison and students age 65 and older. Eligible persons who have not received an email from UHS to their wisc.edu account should send an email to [email protected]. Write ‘vaccinations for vaccine’ in the subject line and give the full name, contact information and a description of the suitability.

Contact your primary care physician if they are not related to the mentioned health systems, for information on how they plan the vaccinations.

How to plan a vaccination appointment at a pharmacy:

More than 175 Walgreens locations in Wisconsin are now planning COVID-19 vaccinations as part of the Federal Retail Pharmacy Program. Go here and click “search for an appointment”, enter your zip code and it will say if there are appointments available. People can also sign up for updates on when appointments are available.

The vaccine is not available at CVS pharmacy locations in Wisconsin.

How to plan a vaccination appointment with local health departments:

Public Health Madison and Dane County are planning appointments for people who do not have a primary care physician. Those who are eligible can complete this form and will be associated with a vaccine.

If people 65 and older need a vaccine ride, they can call New Bridge at 608-512-0000, Extension 3 for a free ride. This service is funded by the Dane County Department of Human Services.
