Vaccination card

When patients receive their first dose at the Department of Health, they receive a vaccination card card with the following important information.

· The vaccine manufacturer (Moderna or Pfizer)

· The date on which the first dose of vaccine was given

· The date on which the second vaccine dose should be

It is important to keep track of the vaccination card, because making an appointment for the second dose of vaccine can ensure that the correct product is received and that it is at the right time. The current recommendation is that vaccinations for the second dose be received on the vaccination card or not before the recommended date.
