Guide to Booking a COVID-19 Vaccine Appointment at Publix

FORT MYERS, Fla. – The next step-by-step process was the process we experienced on 20 and 22 January.

**NBC2 is not responsible if you make changes to the Publix website or the appointment.**

This information will help you as soon as you enter the portal to sign up. Waiting in line can be frustrating, but once you get started, you need to move quickly through the sign-up process.

REMEMBER: You did not secure a place until you arrived at the end of the registration and confirmed.

On the first page you need
– Name
– Mobile phone number
Email Address (this is important because they will send a confirmation email)
– Date of birth
– Address
– Medicare number if you have one
– You will be asked: “Are you a Publix contributor?”

Once the directions are complete, you will be asked to select the store where you want to vaccinate.

REMEMBRANCE: You race with the clock, so know where you want to go and do not be too picky. Just confirm that you are in the right province that you have chosen.

The store will pop up and you will get different options for your appointment. Choose fast and do not waste your time.

You will have to answer 14 questions about your health. Here they are literally. Please read now so you can go through the process quickly:

  1. Contact name for emergencies
  2. Number contact number
  3. Are you 18 years or older?
  4. Do you have a cold, fever or acute illness?
  5. Do you have allergies to medicine, food or any vaccine? If so, please list.
  6. Are you allergic to chicken eggs or egg products?
  7. Are you allergic to Thimerosal (cleaning agent or contact lens solution)?
  8. Have you ever had a serious reaction after receiving a vaccination?
  9. Have you ever been diagnosed with Guillain-Barre syndrome?
  10. Do you have an attack, brain or nerve problem?
  11. Are you the patient? If not, enter your name and relationship with the patient.
  12. Are you visiting a clinic? If so, enter the location?
  13. Do you want to be notified of other vaccinations?
  14. Have you already received an initial dose of the COVID-19 vaccine?

If you take the time you have chosen, you may be asked to choose a new time. If the selected store no longer has appointments, you may be asked to select another store.

After securing your time slot, you are not done. You will get a page to review your information. You should also click two boxes at the bottom of the page. The first is a consent box in which you are the patient or the patient’s representative. The second is a box that says, “I’m not a robot.”

Click Continue and you will see a screen that says “Thank you, (your name), your appointments have been booked” and an appointment ID number. It contains the dates and times of your first and second appointment. It will also have the address of the store you have chosen to do the vaccination.

Print the last screen as security and then check your confirmation email.

THERE IS ANOTHER STEP IF YOU HAVE TO MAKE AN APPOINTMENT FOR ANOTHER PERSON !!!!

At the bottom of your confirmation page there is a link titled “Book another appointment.”

You are now in the system and can book an appointment with another person without standing in line again.
