47-year-old COVID-19 patient dies after ventilator tube is disconnected

The Sourasky Medical Center in Tel Aviv said on Saturday night that it would reduce the number of general patients receiving it and the staff needed to treat them proportionately to better support the needs of the coronavirus units.
The announcement comes after a 47-year-old Moshe Harazy from the Yad Eliyahu neighborhood in Tel Aviv died in the coronavirus intensive care unit at the hospital on Friday night when his ventilator’s breathing tube came loose and staff did not catch it in time.

According to the hospital, his condition worsened and he had to need breathing, which the staff provided even though he was still dead.

Later, an investigation that took place on Sabbath revealed that the man’s ventilator’s breathing tube tore out of place without the staff realizing it in time, the hospital said.

“The hospital management has started an investigation into the unusual incident and will draw the necessary conclusions,” reads a statement by Sourasky, noting that staff ‘in the intensive care unit face several simultaneous warnings’ and that’ staff busy was to respond to other patients. ’emergency warnings and could only evacuate [and head to the 47-year-old patient] after a few minutes. ‘

If machines stop working or loosen, there is a loud noise and other warnings to notify staff of the problem. As noted, staff apparently could not reach the patient in time.

Hospitals have been overcrowded in recent weeks with an increasing number of serious coronavirus patients, which peaked last week. Nearly 2,000 people have been treated for coronavirus in Israel’s hospitals since Saturday night, including more than 1,000 in a serious condition, including 300 people who are intubating.

“This is an excellent ward,” the hospital said in defense of its staff. “The hospital management supports the work of dedicated staff working day and night to care for the most serious coronavirus patients.”

The man left behind a wife and five children. According to the hospital, the family was informed about the details of the incident.
